Understanding Ovulation and the Fertile Window

Ovulation is the release of a mature egg from one of the ovaries. It's the only time each cycle when conception is possible. Contrary to a common misconception, ovulation does not occur 14 days after your period starts β€” it occurs approximately 14 days before your next period. For a 28-day cycle, that's day 14; for a 32-day cycle, it's day 18.

The Fertile Window Explained

The fertile window spans the days when intercourse can result in conception. It includes:

  • 5 days before ovulation (sperm can survive this long in the reproductive tract)
  • The day of ovulation itself (the egg survives ~12–24 hours after release)

Peak fertility occurs in the 2 days before ovulation and on the day of ovulation. The fertile window is 6 days total but is most concentrated in the final 3 days.

Confirming Ovulation: Beyond Calendar Tracking

MethodAccuracyNotes
Calendar/rhythm methodModerateUnreliable for irregular cycles
LH surge test (OPK)HighDetects LH surge 24–48 hrs before ovulation
Basal body temperature (BBT)ModerateConfirms ovulation occurred, not predicts it
Transvaginal ultrasoundVery HighClinical standard; visualizes dominant follicle
Cervical mucus monitoringModerateChanges to egg-white consistency near ovulation

Factors That Disrupt Ovulation

Stress, extreme weight changes, intensive exercise, PCOS (polycystic ovary syndrome), thyroid disorders, and certain medications can all cause irregular or absent ovulation. If you're trying to conceive and have irregular cycles, a gynecologist can evaluate ovulatory function with blood tests and ultrasound.
